Pre-participation Medical Examination
Print and take below form to physician for a Pre-participation physical. Have physician complete and submit to the Student Health Services at Nazareth College.

Sickle Cell Information
See Policies and Procedures under Sports Medicine Heading for Sickle Cell information and waiver form. Waiver to be signed only if you are not submitting proof of testing and you will be under 18 years of age at time attending school. A parent/guardian must also sign waiver. The signed form should be submitted to Student Health Services at Nazareth College prior to attending school.
Medical-Insurance information
As a new student-athlete you are required to register on our electronic medical records system, Athletic Training System,(ATS). See Policies and Procedures under Sports Medicine Heading for Medical-Insurance Information for further details. You can access ATS via the link below. Fix needed to enter into the ATS Web Portal
If you are having issues getting into the ATS please use the following 'fix' to gain access: Internet Explorer 10/11 has some issues that cause some web pages (including ATS) not to work. Below are the steps you/your athletes should follow to correct this issue:
- Bring up the web site first but do not login (for example yoursitename.atsusers.com) Press the "alt" key on the keyboard. This will open the Menu Bar in your browser, then click "Tools"
- Select “Compatibility View” under the tools selections and you should see the page resize
- You should be able to login
Non ADD/ADHD Medication and ADD/ADHD Medication Information
See Policies and Procedures under Sports Medicine Heading for NCAA Drug Policy and appropriate forms to be printed and completed by treating physician. Forms and appropriate documentation needs to be submitted to the Athletic Training Department by August 1.
Authorization for Treatment of Minors
If you will be under 18 when you attend Nazareth, please print form below and have parents or legal guardians complete the form and submit to the Athletic Training Department by August 1.
